My online booking and supplement ordering system is for Established Patients and New Patients who have already had a discovery call and are ready to schedule their first appointment.

Please use the Patient Portal button below to request an appointment through the Charm Patient Portal. We will confirm your request within 48 hours to finalize your appointment time.

If you’re a New patient, please choose “New Patient Visit” to get started. You will need to complete your registration in the Charm Patient Portal before you can book an appointment online.

Returning patients can select from available follow-up options:

  • Standard Follow-up, 30 min, Video/Zoom or Phone

  • Expanded Follow-up, 60 min, Video/Zoom or Phone
